Are you allergic to trimethoprim or septrim (co-trimoxazole)?

Do you suffer from any blood disorders including anaemia and folic acid deficiency?

Do you suffer from kidney damage or are you undergoing dialysis treatment?

Are you taking any of the following medications: existing antibiotics, Amiodarone, Anticoagulants (eg. Warfarin), Ciclosporin, Digoxin, Phenytoin, Pyrimethamine, Azothioprine?

Are you pregnant or breast-feeding?

Yes No

Cystitis symptoms should improve within 24 hours and resolve within 3 days of starting trimethoprim. If symptoms do not improve within 24 or are not resolved within 3 days, please confirm you understand you must see a doctor for further medical assessment.
